Initial Visit
During your first visit at Cityscape Dental, a thorough examination will be completed. This exam will include an oral cancer screening, diagnostic radiographs, intra-oral photos, and cavity detection. This information will allow us to make an accurate diagnosis of your oral conditions. After the exam, we will discuss any recommendations and answer any questions you may have. The key to our successful relationship is an open line of communication.
Our dental hygienists will perform a full general cleaning or begin any recommended therapy for treating gum and periodontal disease. They will also provide oral hygiene instructions, along with suggestions to help you care for your teeth.
Making sound decisions about your dental care and oral health is an easy thing to do with the right preparation beforehand:
Make a list of questions to ask our office, so you don’t forget anything on the day of your appointment. This includes:
- Concerns you have or oral problems you’ve been experiencing.
- If you have dental insurance, remember to bring your insurance card with you.
- If any treatment is recommended, we will discuss options and benefits with you before proceeding. We will work within an established budget, so you obtain the care you desire in an affordable fashion.

New Patient Forms
To prepare for your upcoming appointment at Cityscape Dental, our dental office provides our patient forms online. Completing these forms before your visit will help ensure your timely check-in for your appointment. Please complete your new patient paperwork prior to your appointment. If you are unable to complete the forms online, we ask that you arrive for your appointment 15 minutes early, so you may complete them in the office.
Insurance
Dental insurance is intended to cover some, but not all of the costs of your dental care. Many plans cover preventative care and apply a deductible and co-insurance for routine dental work, payable at the time of service.
At Cityscape Dental, we can file most insurance claims for you. Please bring your insurance plan information with you during your first visit. We will work with you to ensure that you receive the maximum benefits to which you are entitled. If you have coverage questions specific to your plan, check with your human resources coordinator or your insurance enrollment package. Reimbursement amounts are not, and have never been, a guide for quality care.
We are considered in-network for Delta Premier, Metlife, Cigna, and Guardian. We also accept many other traditional dental plans. For more information about insurance options, please call our office and a team member will be more than happy to answer any questions that you may have.
We accept payment for the portion of costs not covered by dental insurance in the form of cash, personal check, and credit cards such as Visa, Mastercard, Discover, and American Express.
We request that you give us 24 hours’ notice if you need to cancel or reschedule an appointment in order to avoid a cancellation fee.
Financing
At Cityscape Dental, we know the cost of obtaining a brilliant and beautiful smile is sometimes limited by the ability to pay for certain dental procedures. We partner with well-known companies to offer financing options so you can achieve the smile of your dreams.
Knowing your financing options for dental treatments can help make your decision about visiting us and starting on your recommended treatment plan. We offer numerous financing solutions to fit any budget and meet your needs. We accept Visa, MasterCard, Discover, and American Express. In addition, we offer third-party financing options through CareCredit. Discounts are also available for services when paid at the time of service or prepayment of services.
Please ask a member of our staff to review your payment options to accommodate your financial needs. If special arrangements are needed, please talk to our office manager prior to receiving service. We will be glad to assist you in any way we can.
